About Resource Coordination
The All Hands Health Network (AHHN) can help you and your family access the resources you need to stay healthy. If you live in 60651 or 60639, you may be eligible for free, short-term, personalized, individual care and support from one of our resource coordinators.
Our Resource Coordinators are trained to best serve you and your family. If you enroll, you will have regular check-ins intended to support you and your family. Some of our services include:
- Helping you address concerns about you or your child’s behavior or wellbeing
- Helping you navigate the school system or other community agencies
- Helping you connect to a community, state or federal programs and resources
- Helping you select a primary care provider or locate specialty care
