The All Hands Health Network (AHHN) is proud to introduce our Socially and Clinically Integrated Network—the first of its kind. Our network is uniquely comprised of three key pools of direct service providers, ensuring comprehensive and holistic care for our community:

Healthcare Providers:

APM Principles: Aligning with AHHN’s Vision

Our Alternative Payment Model (APM) principles are designed to ensure simplicity, transparency, and fairness across the network:

  • Balanced Approach: Maintain simplicity, transparency, and fairness.
  • Performance-Based Rewards: Recognize and reward providers who demonstrate effective management and high performance.
  • Contract Alignment: Reflect key contract themes.
  • Strategic Goals: Incorporate AHHN’s overarching goals and priorities.
  • Provider Accountability: Ensure every provider has at least one efficiency or quality measure.
  • Annual Review: Reevaluate the incentive structure on an annual basis.

EpicCare Link: Connecting Care Providers to Lurie’s EMR

EpicCare Link is a secure, web-based application that grants our external care providers read-only access to their patients’ health records within Lurie’s electronic medical record (EMR) system. This tool enhances collaboration and ensures continuity of care by providing up-to-date patient information.

Behavioral Health Consultation Model

The All Hands Health Network's Behavioral Health Consultation Model is a flexible, case-based system designed to support our network’s PCPs in delivering quality mental health care. Through PH Connect, our secure Provider-to-Provider Communication System, network PCPs can directly communicate with AHHN’s Child Psychologist for assistance with behavioral health (BH) diagnosis, medication management, and evidence-based psychotherapeutic treatment recommendations.

Two Levels of Behavioral Health Support:

Level 1 - Provider Consultation Direct Messaging:

  • Service: Provides primary care clinicians with clinical responses to inquiries regarding the diagnosis and treatment of pediatric ADHD, anxiety, and/or depression.
  • Privacy: This communication method does not require Protected Health Information (PHI).

Level 2 - Consultative Assessments:

  • Service: Offers a one-time, 1:1 virtual evaluation assessment between AHHN’s Child Psychologist and the referred patient to confirm a diagnosis.
  • Privacy: This service requires client-specific background and information.
